Jenkins quick guide

Forms:

-Male history form (for everyone)

-Sexual function form (for ED, low T, Peyronie's, orgasmic dysfunction)

-Vasectomy form (for vasectomy)

-Fertility forms (for fertility evaluation)

ED:

• Does not do viagra 50mg. Only viagra 100mg or cialis 20mg. Likes to do largest dose and titrate back if anything.
• Cialis 5mg daily with viagra 50mg PRN (100mg tablet cut in half) for combo therapy
• He still mentions intraurethral suppositories for educational purposes. He does not have a secret connect
• Uses trimix for ICI, not alprostadil. This is available at the VA too (nonformulary)
• Uses trimix for doppler studies. He performs his own dopplers

EVALUATING LOW T:

• Dr. Jenkins likes to check peaks (24-48 hours after injection) versus Dr. Hellstrom likes to check for troughs (days 6 or 7)
• Takes 7-10 days for the free testosterone lab to come back

Initial evaluation:

Labs to be done before 10am

• LH
• Free testosterone (ENSC) - this is a send out. DO NOT ORDER "TESTOSTERONE FREE PANEL"
• Total testosterone - can be ENSC or in house
• Estradiol
• PSA (only baseline or if clinically indicated)
• CBC w/o differential (he doesn't want the extra info)

Repeat evaluation (done every 6 months once stable):

• LH
• Estradiol
• Free testosterone (ENSC) - send out lab. DO NOT ORDER "TESTOSTERONE FREE PANEL"
• Total testosterone - can be ENSC or in house
• CBC w/o differential (he doesn't want the extra info)

If not stable, recheck the above^ in 3-4 weeks

• PSA if clinically indicated

Treatment:

• Because there are send out labs above, patients need follow up 2-3 weeks after labs are done. Please emphasize this to patients, otherwise they will come to clinic with not all labs back and have to be rescheduled.
• Dr. Jenkins does not refill TRT if there is not a lab check within the last 6 months. If there are no labs back, he will give 1 month refill for patient to have labs done and follow up.
• For testosterone cypionate, he likes twice weekly injections. He may be moving away from this to weekly subq. NEVER EVER EVERY TWO WEEKS.
• Xyosted, he starts with 75mg/weekly doses

FERTILITY:

Initial labs:

• FSH
• LH
• Free testosterone (ENSC) - this is a send out. DO NOT ORDER "TESTOSTERONE FREE PANEL"
• Total testosterone - can be ENSC or in house
• Estradiol
• Semen analysis x2

Treatment:

• Clomid 50mg MWF
• Anastrazole +/-
• hCG +/-
